Comprehensive Eye Exams will be provided to your child per your consent.  If needed, the child will pick out a pair of glasses which will be provided to you at no cost.  In addition, if further testing is needed 

The time you are choosing is not necessarily going to be the time that your child will be seen.  We are doing this to allow the use of electronic sign ups as an easier way to acquire your consent for your child to participate in the program

Please note, if you are booking multiple children, you will have to book an appointment for each child. 

If applicable, vision services provided will be billed to your child’s Medicaid benefits. There will be no out-of-pocket cost.

Tenga en cuenta que si reserva varios niños, deberá reservar una cita para cada niño.

Si corresponde, los servicios de la vista proporcionados se facturarán a los beneficios de Medicaid de su hijo. No habrá ningún costo de bolsillo.
