PLEASE ONLY SIGN UP FOR SITES THAT ARE FITTING FOR YOUR SITUATION.  BCPR SITES ARE  OPEN TO THOSE REGISTERED FOR CAMP AT THAT SITE ONLY.

A comprehensive eye exams will be provided to your child (ages 4-17) per your consent. Your child will have the opportunity to pick out a pair of glasses, at no cost to you, if they receive a prescription. If your child needs further testing, they will receive a referral for follow care.

Please note, if you are booking multiple children, you will have to book an appointment for each child. 

If applicable, vision services provided will be billed to your child’s Medicaid benefits. There will be no out-of-pocket cost.

Se le realizará un examen ocular completo a su hijo (de 4 a 17 años) con su consentimiento. Su hijo tendrá la oportunidad de elegir un par de anteojos, sin costo para usted, si recibe una receta. Si su hijo necesita más pruebas, se le derivará a un centro de seguimiento.

Tenga en cuenta que si reserva varios niños, deberá reservar una cita para cada niño.

Si corresponde, los servicios de la vista proporcionados se facturarán a los beneficios de Medicaid de su hijo. No habrá ningún costo de bolsillo.
